Since Sarah hadn't heard Karyn's wish, her memories were altered so that she remembered that she made a wish for herself to be pregnant and be in the same situation that Karyn was in. She didn't understand why she would wish that on herself, she only knew that she had.
Karyn had become a maid that was employed by Sarah's parents. Since Karyn did hear the wish, she still retained her own unaltered memories, but she found that she had new memories in addition to her old ones. She now remembered being Sarah's maid and she also remembered driving Sarah to the hospital when she was going into labor. She cared a great deal for Sarah, which was a shock to Karyn, since as her original self, she loathed Sarah. As much as she wanted to stay as her original self, it seemed as if her new "maid" self was becoming more dominant.
The doctor asked if Karyn would leave the room, but Karyn insisted to stay, so she could comfort her Mistress Sarah.
