Sarah uncurled from her protective ball, still clutching the stone that Biff had given her before he had gone crazy and attacked her, and looking him straight in the face said "I said, I wish your fucking balls would fall off you raping mother fucker!"
A moment of dread realization of what Sarah's wish meant for him and Karyn passed through Jon's mind as he felt himself suddenly become detached from Biff and half-roll half-slide out of Biff's boxers and down the legs of his pants.
Biff meanwhile only felt a slight tingle in his crotch and then it felt like there was something warm and fleshy moving down each of his pant legs which he tried to ignore. He slapped Sarah once really hard for what she had just said to him not yet realizing that her wish had come true.
Sarah noticed immediately as first Jon and then Karyn came rolling out of the cuffs of Biffs pants. She recognized them right away for what they were: Biff's balls! She didn't know how but her wish seemed to have been granted.
"What the hell?" Biff said seeing the oblong pieces of flesh laying at his feet. "What are those?"
"These?" Sarah said gesturing to the two balls sitting on the ground as she got to her feet. "Why Biff dear, those are your balls." She said with a sneer.
"No way." Biff said noticing that the blobs of flesh did resemble balls but they couldn't possibly be his because his were right here... Biff's hand which had been slowly feeling around his crotch couldn't seem to find his balls. "How did you... what did you... what are you gonna do to me?" Biff finally managed to say the shock draining all color from his face.
"Oh, I don't know; but, I think the punishment should fit the crime don't you Biff? And I don't think that merely losing your balls is enough punishment for what you did."
