"How can that be?" said Karyn.
"Something's wrong," said Jon. "I'm still pushing. I'm still pregnant..."
"Of course you are," said the doctor.
Some time later, Jon pushed hard for a final time and the doctor said "Congratulations again! Your boy has a twin sister!"
"Well," said Jon, "that settles the matter of how Emily can be a boy... she isn't."
"But we're still not finished," said Karyn. "I wish Jon and I and the twins were at home. I also wish Jon wasn't drugged or groggy."
The world shifted around them and they were back. "I'm not finished," explains Karyn. "We can't get completely back to where we were until Emily makes the wish... and she can't like she is now..."
